Mitochondrial dysfunction in Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS): are autonomic phenotypes necessary to clear conflicting results?
Wat bieden we?
- Een voltijds mandaat als doctoraatsonderzoeker in de Pain in Motion onderzoeksgroep van de Vrije Universiteit Brussel en de Katholieke Universiteit Leuven. Je zal ook nauw samenwerken met Newcastle University (Newcastle, UK) en Nicolaus Copernicus University (Torun, Polen).
- Inhoudelijke expertise & internationaal research netwerk.
- Onderwerp doctoraat & uitgewerkte design van de studies.
- PhD training en intensieve begeleiding.

- Je zal voor je doctoraat een klinische studie uitvoeren (patiënten rekruteren, verschillende klinische metingen) en bloedstalen hiervan analyseren in het labo.
- Contractuele aanstelling als doctoraatsonderzoeker met een aanstellingspercentage van 100% voor een periode van 1 jaar, met de mogelijkheid voor een 3-jarig doctoraatstraject in geval van gunstige evaluatie. Je zal ook nodige beurzen aanvragen voor bijkomende financiering ten einde een 4-jarig doctoraatstraject mogelijk te maken.
Profiel:
- Je bent in het bezit (of zal dit behalen vóór september 2023)van een diploma Master of Science in een domein dat relevant is voor dit project (zie hieronder voor een korte beschrijving van het project) (vb: biomedische wetenschappen, biochemie, etc.).
- Je bent intrinsiek gemotiveerd voor het doctoraatsonderwerp (zie hieronder) en werkt nauwkeurig.
- Je beheerst het Engels en bij voorkeur Nederlands en beschikt over een academische schrijfstijl.
- Je bent communicatief vaardig en kan goed in teamverband samenwerken.
- Je bent bereid om te kandideren voor FWO aspirant mandaat.

The only way to improve the diagnosis and treatment of Myalgic Encephalomyelitis / Chronic Fatigue Syndrome (ME/CFS) is to better understand the mechanisms underlying its pathophysiology. Mitochondrial dysfunction has been proposed as a potential pathophysiological mechanism because the mitochondria are important for energy metabolism. Moreover, patients with mitochondrial diseases experience similar symptoms as patients with ME/CFS, including fatigue, pain and autonomic symptoms. However, literature reports conflicting results regarding mitochondrial dysfunction in patients with ME/CFS. As ME/CFS patients present with a broad range of symptoms, subgrouping them might provide new insights.
Next to mitochondrial dysfunction, the involvement of the autonomic nervous system (ANS) has been proposed as it can be linked to common symptoms such as autonomic symptoms, post-exertional malaise and cognitive impairments. Recently, ME/CFS patients were subgrouped into autonomic phenotypes. Although the nature of the link between mitochondria and the ANS remains unclear, a recent animal study showed that activation of adrenergic receptors – the binding sites of catecholamines (i.e. messengers of the ANS) – increased the production of reactive oxidative species, which are in turn known to influence autonomic function, damage the mitochondria, and regulate mitochondrial DNA methyltransferase (DNMT) expression. Interestingly, Buyn et al. reported that heart rate variability, an important marker to subgroup patients with ME/CFS into autonomic phenotypes, was associated with mtDNAm.
This raises the question whether mitochondrial dysfunction in patients with ME/CFS depends on to which autonomic phenotype they belong and whether this relation can be explained by mtDNAm, which is influenced by reactive oxygen species production upon adrenergic receptor activation. Consequently, this relation may lead to autonomic phenotype-dependent mitochondrial dysfunction.
We will therefore enrol 75 patients with ME/CFS and 15 healthy controls into a cross-over study. All participants will undergo one session of aerobic exercise, and an orthostatic test to induce post-exertional malaise. Additionally, participants will be assessed before, during and after the procedures. The assessments include a clinical assessment through several questionnaires, measurement of pain thresholds, blood withdrawal and a comprehensive profiling of autonomic parameters. We aim to subgroup patients with ME/CFS based on the functioning of their autonomic nervous system, and investigate autonomic phenotype-dependent mitochondrial dysfunction.
